New Patient Forms
At the time of your first appointment, we ask that you arrive 20 minutes early so that our registration staff may complete the intake process. We will ask you to complete a medical history, scan your insurance card, and take your picture for your electronic medical record. If you are coming for a consultation and have a managed care insurance plan, please be certain your formal referral has gone through prior to the appointment. Please refer to the list of recommended records (see below) to obtain prior to your appointment for a thorough cancer consultation.
