Often in their mid-40s, people assigned female at birth enter the perimenopausal phase: a time of change in hormone levels and the menstrual cycle as the body prepares for menopause.
It’s normal to feel unlike yourself during this time. According to the American College of Obstetricians & Gynecologists (ACOG), about 4 in 10 women experience mood symptoms during perimenopause similar to PMS that can occur with no pattern or relation to their menstrual cycle. This is known as perimenopausal mood instability. Your emotional symptoms are valid and normal as your body kicks off the rollercoaster to menopause.
How Does Perimenopause Affect Hormones?
During perimenopause, the ovaries decrease in size and release less estrogen. Fluctuating estrogen levels can be paired with several physical and mental health symptoms:
- Irregular periods
- Mood changes
- Hot flashes
- Trouble sleeping
- Vaginal dryness
- Bladder issues
- Changes in sexual desire
- Fluctuations in blood cholesterol levels
While these symptoms are common, it is important to monitor and communicate them to your provider. They can provide more information about possible relief options and spot any abnormalities in your perimenopause symptoms.
Is An Irregular Period Normal During Perimenopause?
Yes. Expect to experience changes in your menstrual cycle during perimenopause. Early on, cycles may become shorter than usual or you may even skip a period. As it progresses, period length and time between periods may vary. It is common to see changes in bleeding as well.

How Do I Know When I’ve Reached Menopause?
While their symptoms overlap, perimenopause and menopause are told apart by looking at your menstrual cycle. The day you reach one full year without your period marks the start of menopause. If the period persists, it’s a sign that you are still in the perimenopausal stage.
